Fix preview latex

* lisp/org.el (org-toggle-latex-fragment): Fix previewing nested
  environments.
(org-format-latex): Change signature.  Rewrite using Elements.

* lisp/ox-html (org-html-format-latex):
* lisp/ox-odt (org-odt--translate-latex-fragments): Apply signature
  change.
