Subject: [PATCH 5/5] Control insertion point for footnote definitions during pre-process.

* lisp/org-footnote.el
(org-footnote-insert-pos-for-preprocessor): New variable.
(org-footnote-normalize): Use it.

definitions be available before any references to them. Such
