This commit documents and unifies previously undocumented assumptions
about the values of `org-time-stamp-formats' and

calculation to `org-time-stamp-format'. The undocumented assumption
about brackets in user option `org-time-stamp-custom-formats' is not
relaxed making the docstring correct.
